Localized sclerosing cholangitis?

Background:

Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by intrahepatic and extrahepatic bile duct inflammation and fibrosis.
Localized strictures of the extrahepatic bile duct are an uncommon presentation of PSC, often leading to misdiagnosis.
Obstructive jaundice is a common symptom requiring prompt investigation and accurate diagnosis.

Observation:

A 56-year-old male presented with obstructive jaundice.
Endoscopic retrograde cholangiography revealed a localized, smooth stricture in the common hepatic duct, initially suspected as cholangiocarcinoma.
Histological examination of resected tissue and a later liver biopsy confirmed PSC affecting both intrahepatic and extrahepatic bile ducts.

Findings:

The patient's condition, initially mimicking cholangiocarcinoma, was histologically confirmed as primary sclerosing cholangitis.
The presentation of PSC as a localized extrahepatic stricture is rare but possible.
The disease progression involved both the extrahepatic and intrahepatic biliary tree.

Implications:

This case underscores the importance of considering PSC in the differential diagnosis of localized biliary strictures, even with initial suspicion of malignancy.
Accurate histological diagnosis is crucial for appropriate management of biliary strictures.
Understanding rare presentations of PSC can improve diagnostic accuracy and patient outcomes in hepatobiliary diseases.
