    Background:

    • Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by intrahepatic and extrahepatic bile duct inflammation and fibrosis.
    • Localized strictures of the extrahepatic bile duct are an uncommon presentation of PSC, often leading to misdiagnosis.
    • Obstructive jaundice is a common symptom requiring prompt investigation and accurate diagnosis.

    Observation:

    • A 56-year-old male presented with obstructive jaundice.
    • Endoscopic retrograde cholangiography revealed a localized, smooth stricture in the common hepatic duct, initially suspected as cholangiocarcinoma.
    • Histological examination of resected tissue and a later liver biopsy confirmed PSC affecting both intrahepatic and extrahepatic bile ducts.

    Findings:

    • The patient's condition, initially mimicking cholangiocarcinoma, was histologically confirmed as primary sclerosing cholangitis.
    • The presentation of PSC as a localized extrahepatic stricture is rare but possible.
    • The disease progression involved both the extrahepatic and intrahepatic biliary tree.

    • This case underscores the importance of considering PSC in the differential diagnosis of localized biliary strictures, even with initial suspicion of malignancy.
    • Accurate histological diagnosis is crucial for appropriate management of biliary strictures.
    • Understanding rare presentations of PSC can improve diagnostic accuracy and patient outcomes in hepatobiliary diseases.
