Background:

  • Ruminococcus gnavus is a normal inhabitant of the human gut microbiota.
  • Intestinal dysbiosis associated with chronic conditions and immunosuppression can lead to bacterial translocation.
  • Ruminococcus gnavus bacteremia is a rare but serious complication.

Observation:

  • A case of Ruminococcus gnavus bacteremia is presented in an elderly male with diverticular disease and moderate SARS-CoV-2 infection.
  • The patient required corticosteroid therapy for SARS-CoV-2 and experienced a prolonged hospital stay with multiple infections.
  • A literature review identified 17 previously reported cases of Ruminococcus gnavus bacteremia.

Findings:

  • Common predisposing factors in reported cases include gastrointestinal issues (bleeding, diverticular disease, colitis, cholecystitis, fistula) and orthopedic prosthesis infections.
  • A history of hemato-oncological disease and corticosteroid use was noted in another case.
  • The patient's diverticular disease and SARS-CoV-2-induced immunosuppression from corticosteroids likely contributed to the Ruminococcus gnavus bacteremia.

Implications:

  • This case underscores the potential for commensal gut bacteria like Ruminococcus gnavus to cause invasive infections under conditions of immunosuppression.
  • Clinicians should consider Ruminococcus gnavus as a potential pathogen in patients with relevant comorbidities and compromised immune status.
  • Further research into the factors promoting Ruminococcus gnavus translocation and subsequent bacteremia is warranted.
