Summary

The sinus tarsi approach (STA) offers similar complication and revision rates compared to the extended lateral approach (ELA) for intra-articular calcaneal fractures. Both surgical methods provide comparable radiologic and functional outcomes in the short to midterm.

Background:

Purpose of the Study:

  • To evaluate the complication and revision rates of the sinus tarsi approach (STA) compared to the extended lateral approach (ELA).
  • To assess the impact of STA versus ELA on radiologic and functional outcomes in patients with intra-articular calcaneal fractures.

Main Methods:

  • Retrospective cohort study of 241 patients with intra-articular calcaneal fractures treated with ELA or STA.
  • Propensity score matching was used to control for confounding variables, resulting in 140 analyzed cases.
  • Primary endpoint: time to surgical revision; Secondary endpoints: complication rates, operating room time, length of stay, and functional outcomes (SF-12, VAS-FA).

Main Results:

  • No significant difference was found in the hazard ratio for surgical revision (1.74) or complications (1.66) between ELA and STA.
  • Radiologic outcomes, including remaining step, gap, and reduction grading, were comparable between the two surgical approaches.
  • Functional outcomes, assessed by SF-12 and VAS-FA scores, showed no significant difference between patients treated with ELA or STA.

Conclusions:

  • The sinus tarsi approach (STA) is a less invasive technique with similar short- to midterm complication and revision rates compared to the extended lateral approach (ELA).
  • STA provides comparable anatomical reduction and functional outcomes to ELA for intra-articular calcaneal fractures.
  • STA is a suitable surgical option for moderate to severe Sanders type 3 and 4 calcaneal fractures.
