Summary

Fam-trastuzumab deruxtecan-nxki (T-DXd) is approved for HER2 IHC 3+ tumors. This study found HER2 IHC 3+ lung cancers often lack ERBB2 alterations but have other mutations, and preanalytical factors affect testing.

Background:

  • Limited data exists on HER2 IHC testing and genomic correlates in lung cancer.

Purpose of the Study:

  • To analyze genomic characteristics of HER2-expressing lung tumors.
  • To investigate preanalytical variables impacting HER2 IHC assessment in lung cancer specimens.

Main Methods:

  • HER2 IHC staining on archival lung cancer cytology and surgical pathology specimens.
  • Correlation of patient/tumor characteristics and next-generation sequencing (NGS) data with HER2 IHC results.

Main Results:

  • 13% of 166 thoracic tumor samples showed HER2 IHC 3+.
  • HER2 IHC scores were lower in cytology compared to surgical specimens (79% decrease).
  • Only 14% of HER2 IHC 3+ cases with NGS had ERBB2 alterations; most (81%) had non-ERBB2 mutations (KRAS, TP53, STK11).

Conclusions:

  • HER2 IHC 3+ lung cancers represent a small, clinically relevant subgroup.
  • These tumors frequently harbor non-ERBB2 genomic alterations.
  • Preanalytical variables, such as specimen fixation, significantly influence HER2 IHC assessment.
