Incorporating Nurse Navigation to Improve Cancer Survivorship Care Plan Delivery

Background:
Survivorship care plans (SCPs) help cancer survivors transition to life after cancer treatment by providing a treatment summary, education, and screening recommendations. Nurse navigators improve care coordination.
Objectives:
This project evaluates the impact of incorporating a survivorship oncology nurse navigator (SONN) into an advanced practice provider-led team to improve social determinants of health (SDOH) screening and SCP delivery at a community-based cancer center.
Methods:
Rates of previsit education, overall SCP completion, advanced practice provider-led SCP visits, and SONN-led SCP visits in the three months before and after the intervention were measured. Completion rates for SDOH screening and the SCP translation rate for patients who spoke a primary language other than English were measured.
Findings:
SCP completion increased from 49 to 159. Patients who received education from the SONN before their SCP visit were most likely to complete it (94% completion). Screening for SDOH improved from 0% to 89%, and all patients who did not speak English as a primary language received the SCP in their primary language after the intervention.
