Background:

  • Major trauma survivors often experience lasting physical, psychological, and cognitive impairments.
  • Return to work (RTW) is a critical indicator of successful reintegration and recovery, not solely health-dependent.

Purpose of the Study:

  • To systematically review and identify factors influencing return to work (RTW) after major trauma.

Main Methods:

  • Systematic review of 12 studies involving 6907 adult major trauma patients (Injury Severity Score ≥ 16).
  • Data were qualitatively analyzed, assessing factors based on the strength of evidence.
  • Risk of bias was evaluated using the 'Quality in Prognostic studies' tool.

Main Results:

  • Numerous factors identified, including physical (injury location), personal (age), and environmental (preinjury income) aspects.
  • Only age, educational level, intensive care unit (ICU) stay, and length of hospital stay (LOS) demonstrated moderate or strong evidence.
  • High risk of bias was noted in 'study confounding' across included studies.

Conclusions:

  • Limited evidence currently supports most identified factors affecting RTW post-major trauma.
  • Younger age, higher educational attainment, and shorter ICU and hospital LOS are associated with better RTW prospects.
  • Further research is needed to strengthen the evidence base for RTW factors.
