Abstract:
Trichophyton indotineae is a fungus that is often antifungal-resistant and presents with exuberant infection. It has been increasingly reported across the world, posing significant diagnostic challenges. We describe a case of a toddler with a history of travel to Bangladesh, presenting with persistent annular scaling plaques of dermatophytosis that were unresponsive to standard antifungal treatments. Misidentification of the Trichophyton species on fungal culture delayed appropriate treatment, with resolution of the rash with a course of itraconazole. This case underscores the need for heightened clinical suspicion and the value of empiric therapy in managing pediatric T. indotineae infection.
Insights
Trichophyton indotineae, an antifungal-resistant fungus, causes challenging infections. Early identification and appropriate treatment, like itraconazole, are crucial for managing pediatric cases.
Area of Science:
- Mycology
- Dermatology
- Infectious Diseases
Background:
- Trichophyton indotineae is an emerging fungal pathogen known for antifungal resistance and severe infections.
- Increasing global reports highlight diagnostic challenges associated with this dermatophyte.
- Pediatric cases require specific consideration due to potential for atypical presentations.
Observation:
- A toddler with travel history to Bangladesh presented with persistent, scaling annular plaques characteristic of dermatophytosis.
- The infection was unresponsive to standard antifungal therapies, indicating potential resistance.
- Initial misidentification of the causative Trichophyton species on fungal culture delayed definitive management.
Findings:
- The case highlights the diagnostic difficulties posed by Trichophyton indotineae, particularly in children.
- Resolution of the extensive rash was achieved with a course of itraconazole, an effective antifungal agent.
- This underscores the importance of considering T. indotineae in refractory pediatric dermatophytosis.
Implications:
- Heightened clinical suspicion for T. indotineae is essential in pediatric dermatology, especially with relevant travel history.
- Empiric therapy with agents like itraconazole may be valuable when diagnosis is uncertain.
- Accurate species identification and appropriate treatment are critical to prevent prolonged illness and spread.
