    Background:

    • Trigeminal neuralgia (TN) is a debilitating neuropathic facial pain condition.
    • Previous treatments for TN include alcohol block, thermorhizotomy, and microvascular decompression.

    Observation:

    • This study evaluated 29 patients with trigeminal neuralgia (2 postherpetic, 27 idiopathic) treated with retrogasserian glycerol injection.
    • The mean age of idiopathic TN patients was 65.2 years, with a mean symptom duration of 7.6 years.

    Findings:

    • Retrogasserian glycerol injection was performed using a 22-gauge needle via the foramen ovale under fluoroscopic guidance.
    • Glycerol (0.15-0.6 ml) was injected into the trigeminal cistern, with cisternography used for needle placement verification.
    • Complications included dysesthesia (81%), hypertension (70%), and hypalgesia/hypesthesia (48%), with most resolving within 8 weeks. Persistent dysesthesia and hypalgesia were noted in 30% of cases.

    Implications:

    • Retrogasserian glycerol injection is a minimally invasive procedure for trigeminal neuralgia.
    • Understanding potential complications and their resolution is crucial for patient management.
    • This technique offers an alternative treatment option for patients with trigeminal neuralgia, particularly those with idiopathic forms.
