    Background:

    • Vascular disease is a common cause of amputation in the elderly.
    • Long-term prosthetic use is crucial for maintaining mobility and quality of life in amputees.
    • Identifying predictors of successful prosthetic use is essential for effective rehabilitation.

    Purpose of the Study:

    • To determine factors associated with successful long-term prosthetic use in elderly patients with vascular disease-related amputations.
    • To compare prosthetic success rates between below-knee amputations (BKAs) and above-knee amputations (AKAs).

    Main Methods:

    • A telephone survey was conducted with elderly patients who had amputations secondary to vascular disease between 1977 and 1982.
    • Success was defined as wearing the prosthesis at least part of every day.
    • Statistical analysis, including chi-square tests, was used to compare success rates and identify contributing criteria.

    Main Results:

    • Below-knee amputees (BKAs) demonstrated significantly higher success rates (37/38) compared to above-knee amputees (AKAs) (7/18).
    • For AKAs, only compliance and post-prescription medical problems significantly differed between successful and unsuccessful long-term users.
    • BKAs had a success rate of over 97%, while AKAs had a success rate of approximately 39%.

    Conclusions:

    • Below-knee amputation (BKA) patients are considerably more likely to achieve successful long-term prosthetic use than above-knee amputation (AKA) patients.
    • Physical therapists can use these findings to set realistic expectations for geriatric amputees.
    • Compliance and managing medical issues post-prescription are critical for successful long-term prosthetic use in above-knee amputees (AKAs).
