    Background:

    • Understanding soleus muscle neurovascular anatomy is crucial for reconstructive surgery.
    • Previous studies have not fully elucidated the independent neurovascular supply to soleus subunits.

    Observation:

    • Dissection and angiography of 86 limbs revealed a consistent bipenniform soleus muscle morphology.
    • Medial and lateral soleus subunits demonstrated independent distal neurovascular pedicles.

    Findings:

    • This bipenniform anatomy permits surgical division into hemisoleus muscle flaps.
    • Thirty-three patients underwent successful hemisoleus flap transfers (medial and lateral, proximal and distal).

    Implications:

    • Hemisoleus flaps preserve ipsilateral soleus muscle function, maintaining plantar flexion strength in ambulatory patients.
    • These flaps offer an extended arc of rotation compared to traditional soleus flaps, enhancing reconstructive versatility.
