    Background:

    • Renal vein thrombosis (RVT) typically complicates nephrotic syndrome in adults.
    • Postpartum RVT is rare, potentially linked to hypercoagulable states in pregnancy and nephrotic syndrome.

    Observation:

    • Two postpartum women without prior renal disease or heavy proteinuria developed RVT.
    • Clinical suspicion of pyelonephritis preceded diagnosis in both cases.

    Findings:

    • Radiologic examination confirmed renal vein thrombosis in both non-nephrotic postpartum patients.
    • Neither patient exhibited significant proteinuria, challenging typical RVT associations.

    Implications:

    • Renal vein thrombosis should be considered in postpartum women presenting with flank pain.
    • This highlights the importance of considering RVT in the differential diagnosis of postpartum flank pain, even without nephrotic syndrome.
