Investigating the Physiologic Role of Leptin and Its Relationship to Obesity

Abstract:
Leptin is a peptide hormone that plays a critical physiologic role in the human body, regulating appetite, food intake, and body weight by entering the central nervous system and acting in the hypothalamus of the brain. Understanding leptin's physiologic role in regulating satiety has raised the question among clinical researchers regarding whether there is a relationship between leptin and the development of obesity. Numerous studies have identified elevated serum leptin levels in patients with obesity, with signs of leptin resistance. However, the mechanism of leptin resistance in obesity remains unclear, and whether there is a causal relationship between leptin resistance and obesity has not been elucidated. Current theories regarding leptin resistance in obesity suggest it may arise due to pathologic changes in the blood-brain barrier, potentially preventing leptin from entering the central nervous system. However, numerous studies have identified other potential mechanisms that may contribute to leptin resistance in obesity. The importance of understanding the mechanisms of this phenomenon stems from the growing need to identify and develop potential therapies to address the obesity epidemic in the United States, as obesity is associated with increased morbidity and mortality and a decrease in quality of life. To this end, researchers have tested leptin analogs in combination with other medications in patients with obesity to determine if any potential benefit exists; while the results of these studies showed some weight-loss benefit, the use of leptin analogs comes with significant obstacles that must be overcome before serious consideration as therapeutic options in the future.
