    • Urine analysis isolated Bacteriodes ureolytcus and Peptostreptococcus asaccharolyticus, indicating bacterial involvement.

    • This case highlights the potential for preputial calculi to cause systemic symptoms like altered consciousness.
    • Management requires addressing both the calculi and the underlying phimosis and bacterial infection.
    • Further research into the pathogenesis and optimal treatment of preputial calculi in cases of severe phimosis is warranted.
