    Background:

    • A novel, 10-inch high step test protocol was designed with audio instructions and stepping rates.
    • Maximal oxygen uptake (VO2max) was directly measured via bicycle ergometry, estimated by the step test, and measured by the authors' bike test in 48 participants (aged 19-70).
    • Heart rates were monitored during the step test using a digital tachograph.

    Main Results:

    • No significant differences in VO2max were observed between the two bicycle protocols.
    • The step test significantly overestimated VO2max by 12% compared to direct bicycle measurement, consistent with expected differences between stepping and cycling.
    • High correlations were found between direct VO2max measurements and both estimates (r=0.92), and between the two estimates (r=0.98).

    Conclusions:

    • The developed step test protocol provides accurate and reliable estimates of maximal oxygen uptake (VO2max).
    • The protocol is safe, brief, and suitable for in-the-home fitness assessment of adults aged 19-70 years.
    • This method is advantageous for conducting large-scale epidemiologic studies requiring convenient fitness evaluations.
