Enhancing Craniofacial Functional Outcomes: A Theory of Planned Behavior-based Voice Rehabilitation Program for

Objective:
The objective of this study is to develop a postoperative voice training management program grounded in the Theory of Planned Behavior (TPB) for patients undergoing vocal cord polypectomy and to evaluate its preliminary efficacy.
Methods:
The program was developed through a systematic process that involved a comprehensive literature review, group discussions, and a Delphi expert consultation process, all aligned with the TPB framework. Using convenience sampling, 82 patients who underwent vocal cord polypectomy at the First People's Hospital of Jingzhou, China, between November 2023 and January 2024 were selected for the study. Participants were divided into a control group (n=41) and an observation group (n=41) based on the sequence of their surgeries. Patients in the control group received routine postoperative voice training, while those in the observation group underwent an additional postoperative voice training management program based on TPB that was specifically designed as part of the study. Key outcomes that were compared between the 2 groups were the objective acoustic voice scores, the voice handicap index (VHI), and vocal cord recovery status.
Results:
During the program development phase, 2 rounds of Delphi expert consultation yielded a 100% questionnaire response rate, with authority coefficients of 0.833 and 0.823. Kendall's coefficients of concordance were 0.312 and 0.401, respectively and the coefficient of variation for each item was ≤0.25 ( P <0.05). Postintervention, patients in the observation group exhibited significantly better outcomes in objective acoustic voice assessments and vocal cord recovery compared with those in the control group, and the VHI scores were significantly lower than those in the control group ( P <0.05).
Conclusion:
The postoperative voice training management program based on TPB for patients undergoing vocal cord polypectomy is scientifically robust and clinically effective. The training was effective in improving vocal cord recovery and alleviating voice disorders in patients undergoing vocal cord polypectomy. And provided new insights for craniofacial surgery nursing.
