    Summary

    Delayed fetal lung fluid clearance, or wet lung disease (WLD), can present with unusual chest X-ray findings. These transient WLD signs may mimic pneumonia but resolve with supportive care, guiding treatment decisions.

    Background:

    • Delayed clearance of fetal lung fluid, known as wet lung disease (WLD), is a recognized clinical and radiographic diagnosis in newborns.
    • Infants with WLD typically present with respiratory distress due to fluid in the lungs.

    Observation:

    • This study reports unusual chest roentgenographic findings in six infants diagnosed with WLD.
    • These infants exhibited delayed development of pleural effusions or transient worsening of pulmonary infiltrates, mimicking infection.
    • Five of the six infants received antibiotic treatment for suspected pneumonia.

    Findings:

    • Despite initial radiographic signs suggestive of pneumonia, all infants were clinically well and radiographically normal by 72 hours.
    • Cultures remained negative, and subsequent radiographs showed resolution of pulmonary opacities and effusions.
    • The observed radiographic changes are proposed to be within the spectrum of WLD.

    Implications:

    • The findings suggest that certain radiographic findings initially suggestive of neonatal pneumonia may actually represent WLD.
    • This highlights the importance of considering WLD in the differential diagnosis of neonatal respiratory distress with atypical radiographic presentations.
    • Early recognition can lead to appropriate management adjustments, including discontinuation of unnecessary antibiotics.
