    Ultrasonography for suspected abdominal aortic aneurysms frequently reveals unrelated intraabdominal issues. A significant number of these unexpected findings included retroperitoneal malignancies mimicking aneurysms.

    Background:

    • Abdominal aortic aneurysms (AAAs) are a significant vascular concern.
    • Ultrasonography is a common diagnostic tool for suspected AAAs.
    • The diagnostic yield of ultrasonography beyond AAA detection is under investigation.

    Observation:

    • A study examined 125 patients for suspected abdominal aortic aneurysms using ultrasonography.
    • 15 patients (12%) presented with intraabdominal abnormalities unrelated to the aorta.
    • 12 of these patients had retroperitoneal malignancies that mimicked aneurysms.

    Findings:

    • In three additional patients, unrelated abnormalities were found alongside an aneurysm.
    • These included a uterine myoma, retroperitoneal lymphoma, and colonic carcinoma with lymphadenopathy.
    • The high incidence of incidental findings underscores the comprehensive nature of abdominal ultrasonography.

    Implications:

    • Ultrasonography is crucial for evaluating suspected abdominal aortic aneurysms.
    • Incidental intraabdominal findings, including malignancies, are common.
    • Comprehensive ultrasound evaluation is recommended for patients presenting with suspected AAAs.
