Corticosteroids are not routinely recommended for viral central nervous system (CNS) infections like meningitis and encephalitis, except possibly for myelitis. Current evidence does not support their use, and further research is needed.
Area of Science:
- Neurology
- Infectious Diseases
- Pharmacology
Background:
- Viruses frequently cause central nervous system (CNS) infections.
- Antiviral treatments may be limited or ineffective, leading to the use of corticosteroids.
- Corticosteroids have anti-inflammatory and immunosuppressive properties.
Purpose of the Study:
- To review the rationale and clinical experience of using corticosteroids as an adjunctive therapy for viral CNS infections.
- To evaluate the efficacy and safety of corticosteroids in managing viral meningitis, myelitis, and encephalitis.
Main Methods:
- Review of existing literature, including population-based studies, observational data, and randomized clinical trials.
- Analysis of the effects of corticosteroids on viral CNS infections based on current evidence.
Main Results:
- Empiric dexamethasone for viral meningitis showed no significant improvement or adverse effects.
- Standard empiric treatment for myelitis involves methylprednisolone, with no strong data to alter this approach for viral myelitis.
- Adjunctive corticosteroids in viral encephalitis have not demonstrated substantial benefits in observational data or clinical trials.
- Observed risks associated with corticosteroid use in these conditions were generally low.
Conclusions:
- Routine use of corticosteroids is not supported for viral CNS infections, with the exception of myelitis.
- Further large-scale, multidisciplinary trials are necessary to establish evidence-based treatment guidelines for conditions like encephalitis.
