Area of Science:

  • Surgical Innovation
  • Medical Imaging
  • Oncologic Surgery

Background:

  • Membrane anatomy (MA) is crucial in surgery for operating within natural planes and minimizing injury.
  • Intraoperative identification of membranous structures is challenging, compromising resection and preservation.
  • There is a need for advanced solutions to improve membrane visualization during surgery.

Purpose of the Study:

  • To develop and validate DeepGuide, an innovative tool for enhanced intraoperative membrane visualization.
  • To assess the effectiveness of DeepGuide in laparoscopic radical resection for gastrointestinal tumors.
  • To improve surgical precision and adherence to membrane anatomy principles.

Main Methods:

  • A single-arm prospective study involving 60 patients undergoing laparoscopic radical resection.
  • Development of the DeepGuide tool for intraoperative membrane visualization.
  • Evaluation of primary outcomes including signal-to-background ratio (SBR), mesenteric integrity, light absorbance, and collagen component analysis.

Main Results:

  • DeepGuide improved mesenteric integrity from 75% to 98% compared to conventional laparoscopy.
  • DeepGuide achieved a higher SBR (2.30 ± 0.15) than conventional lighting (1.32 ± 0.16).
  • Analysis indicated reduced light absorption at 385 nm, correlating with collagen I/III enrichment in the submucosa.

Conclusions:

  • DeepGuide significantly enhances intraoperative membrane visualization without contrast agents.
  • The tool improves surgical precision and supports membrane anatomy principles in oncologic surgery.
  • DeepGuide shows potential to establish new standards in oncologic surgical procedures.
