The case of a newborn with multifocal congenital hemangiomas with an activating pathogenic variant of PIK3CA is presented. This report highlights the difficulty in diagnosing multifocal vascular lesions in newborns. Additionally, this case emphasizes the importance of clinical, histologic, radiographic, and genomic correlation to the best categorize rare vascular lesions and to guide treatment.
