Summary

Chromosomal copy number alterations, specifically chromosome 1p loss and 1q gain, can significantly refine meningioma grading. These genetic markers improve the prediction of progression-free survival and aid in updating the World Health Organization (WHO) classification of central nervous system (CNS) tumors.

Background:

  • The 2021 World Health Organization (WHO) classification for central nervous system (CNS) tumors introduced rare molecular features for meningioma grading.
  • Previous research suggested incorporating chromosomal copy number alterations (CNAs) to enhance meningioma grading systems.

Purpose of the Study:

  • To identify specific chromosomal copy number alterations (CNAs) that can improve the current CNS WHO grading of meningioma.
  • To correlate CNAs with progression-free survival (PFS) to discover novel outcome biomarkers.

Main Methods:

  • A multi-institutional cohort study involving surgically treated meningioma patients.
  • Analysis of chromosomal copy number alterations and their correlation with progression-free survival (PFS) using Cox regression.
  • Data collected from 2016 and analyzed between January and September 2024.

Main Results:

  • Loss of chromosome 1p in WHO grade 1 meningiomas was associated with significantly worse PFS, comparable to WHO grade 2 tumors.
  • Combined loss of chromosome 1p and gain of chromosome 1q correlated with outcomes similar to WHO grade 3 tumors, irrespective of initial grade.
  • A total of 1964 meningioma patients were assessed.

Conclusions:

  • Cytogenetic profiling, particularly chromosome 1p loss and 1q gain, should be considered for future CNS WHO grading iterations.
  • Chromosome 1p loss should be a criterion for CNS WHO grade 2, and 1q gain for CNS WHO grade 3 meningiomas.
  • These findings underscore the prognostic value of specific CNAs in meningioma.
