Summary

Triamcinolone tendon sheath injections effectively treat trigger finger without observed complications when administered at 4 mg doses with intervals of at least one month. This retrospective study confirms the safety and utility of this nonsurgical approach for trigger finger management.

Background:

  • Trigger finger is a common condition affecting the hand's flexor tendons.
  • Triamcinolone tendon sheath injection is a recognized nonsurgical treatment option.
  • Concerns exist regarding potential complications like tendon rupture and infection with excessive administration.

Purpose of the Study:

  • To retrospectively evaluate the outcomes of triamcinolone tendon sheath injections for trigger finger.
  • To assess the safety and efficacy of administering 4 mg triamcinolone without limiting injection frequency, provided intervals are at least one month.

Main Methods:

  • Retrospective analysis of patients with trigger finger receiving triamcinolone injections across four facilities (April 2009 - October 2021).
  • Exclusion criteria included follow-up <3 months and pediatric patients (<16 years).
  • Data collected: Quinnell severity, injection frequency/intervals, complications, diabetes status (HbA1c), and surgical rates.

Main Results:

  • 356 cases (715 fingers) were analyzed; mean age 64.9 years; median follow-up 27 months.
  • Average 3.3 injections per finger with a mean interval of 6.3 months; no tendon ruptures or infections observed.
  • 22.4% of patients had diabetes; 9.8% ultimately required surgery.

  • This approach demonstrated no observed complications in the study cohort.
  • The findings support the continued use of this treatment protocol for trigger finger.
