This study investigates the factors influencing the adoption and sustained use of the Patient Empowerment Platform (PEP) and Personalised Care Plan Management Platform (PCPMP) among patients and healthcare providers. Using the extended Unified Theory of Acceptance and Use of Technology (UTAUT) model, we analysed responses from 22 participants over a two-phase longitudinal study. Data were collected at two time points (at the beginning of the project and 4-6 weeks after) to assess changes in performance expectancy, effort expectancy, social influence, facilitating conditions, technology anxiety, adoption timeline, behavioural intention, and usability factors. Quantitative findings showed minor increases across most UTAUT constructs, with a significant rise in Social Influence, suggesting that peer support plays a role in adoption within healthcare settings. Qualitative analysis revealed areas for improvement, such as the need for integrated notifications, enhanced training, a simpler interface, and greater adaptability for users with varying levels of technological proficiency. Our findings underscore the importance of designing user-centred digital health platforms to enhance usability, promote patient empowerment, and support personalised care. These insights provide a basis for refining digital health solutions to better align with user needs and promote sustained engagement across diverse healthcare environments.
