Purpose of the Study:

  • To investigate the factors influencing the transition from PD to HD in young adults.
  • To identify risks and reasons for PD treatment changes in this demographic.

  • Sequential explanatory mixed-methods study design.
  • Quantitative analysis of 470 participants (1987-2015) using Cox proportional hazards models.
  • Qualitative interviews with 13 young adults (2013-2015, interviewed 2020).

Main Results:

  • Young adulthood (15-30 years) significantly increased the risk of transitioning to HD (HRs 2.41-3.39).
  • Primary causes for transition included infection (50%), noncompliance (21%), and mechanical issues (18%).
  • Qualitative themes: treatment communication, life impact, support systems, and resilience in PD patients.

Conclusions:

  • Young adults face higher risks of transitioning to HD due to transplant failure and PD complications.
  • PD challenges are underestimated; comprehensive education on all dialysis options is needed.
  • Enhanced support, including mental health and peer support, is recommended for young adults on dialysis.
