    Background:

    • Autoimmunity is increasingly recognized as a factor in various gastrointestinal disorders.
    • Diarrheal diseases, including coeliac disease and inflammatory bowel disease, warrant investigation for autoimmune underpinnings.

    Observation:

    • Coeliac disease (CD) patients exhibit significantly higher rates of tissue-reactive autoantibodies compared to healthy and disease controls.
    • A subset of patients with protracted diarrhea and refractory enteropathy show evidence of autoimmune predisposition, including specific gut autoantibodies.
    • Inflammatory bowel disease (IBD) shows evidence of autoimmunity through serum antibodies and cytotoxic T cells targeting colonic epithelium.

    Findings:

    • Autoantibodies in coeliac disease patients are prevalent, with some specific antibodies (R1 antireticulin) resolving on a gluten-free diet, while others persist.
    • Seven cases of a proposed "autoimmune enteropathy" demonstrated refractory small intestinal enteropathy despite total parenteral nutrition, coupled with high-titer autoantibodies and/or associated autoimmune conditions.
    • An unusual pediatric case with chronic diarrhea, villous atrophy, and co-occurring thyrotoxicosis and microscopic colitis responded to gluten withdrawal, TPN, and steroids, suggesting complex autoimmune interactions.

    Implications:

    • The findings support a broader role for autoimmunity in diarrheal diseases, potentially necessitating new diagnostic and therapeutic approaches.
    • Recognition of "autoimmune enteropathy" as a distinct category could improve patient management and outcomes.
    • Further research into the specific autoimmune mechanisms in these conditions is crucial for developing targeted therapies.
