Background:

Breastfeeding decisions are complex and influenced by multiple factors.
The role of fathers in supporting breastfeeding is increasingly recognized.
Understanding paternal influence is crucial for effective breastfeeding support strategies.

Purpose of the Study:

To synthesize qualitative evidence on how fathers influence mothers' breastfeeding decisions.
To explore the nature and extent of paternal involvement in breastfeeding maintenance or interruption.

Main Methods:

Qualitative metasynthesis of 13 studies.
Systematic searches across LILACS, PubMed, Scopus, and CINAHL databases.
Thematic synthesis approach guided by ENTREQ and CASP recommendations.

Main Results:

Paternal influence was categorized into two analytical themes: 'Difficulty in actively participating' and 'Acting directly or indirectly'.
Four descriptive themes emerged under 'Difficulty in participating', and three under 'Acting directly or indirectly'.
The mother's decision-making process was predominantly centered on her.

Conclusions:

Fathers' involvement can significantly impact breastfeeding outcomes.
Prenatal health education targeting fathers promotes participatory fatherhood.
Engaging fathers in education can encourage and support breastfeeding continuation.
