A case of primary meningococcal conjunctivitis in an older patient

  • Treatment with topical cefmenoxime and oral levofloxacin led to symptom resolution and improved visual acuity to 0.8.
  • The identified *N. meningitidis* isolate (ST-11026) was unique to Japan and lacked capsular polysaccharide synthesis, a key virulence factor.

Implications:

  • This case highlights the importance of prompt conjunctival cultures in patients with severe purulent conjunctivitis, irrespective of age, to rule out PMC.
  • Early diagnosis and treatment of PMC can prevent progression to life-threatening IMD.
  • The unique characteristics of the Japanese ST-11026 isolate warrant further investigation into meningococcal pathogenicity and epidemiology.
