This study shows that emotional labor significantly impacts nurses' burnout and work engagement in South Korea's Integrated Nursing-Care Service wards. Interventions should focus on reducing burnout and enhancing work engagement for better nursing care.

Background:

  • The South Korean Integrated Nursing-Care Service (INS) was established in 2016, providing national insurance-covered care, particularly in cancer units.
  • INS wards deliver holistic nursing care without paid informal caregivers, aiming to improve patient outcomes and nursing practice.

Purpose of the Study:

  • To analyze factors influencing nurses' work engagement within Integrated Nursing-Care Service wards.
  • To examine the roles of job demands, job resources, and personal resources on nurses' burnout and work engagement using Demerouti's Job Demands-Job Resources Model.

Main Methods:

  • A quantitative survey was conducted with 375 nurses in three tertiary hospitals operating INS wards from April to June 2022.
  • Structural equation modeling was used to test 32 hypotheses regarding the direct and indirect effects of exogenous variables (job demands, job resources, personal resources) on work engagement, with burnout as a mediator.

Main Results:

  • Burnout partially mediated the relationship between job demands/resources and work engagement.
  • Emotional labor had the highest impact on nurses' burnout, followed by resilience and satisfaction with recognition.
  • Nurses' burnout, work environment, emotional labor, work overload, and resilience significantly influenced work engagement.

Conclusions:

  • Emotional labor and burnout are critical factors affecting nurses' work engagement in INS wards.
  • Resilience and satisfaction with recognition also play significant roles in mitigating burnout and enhancing engagement.
  • Findings provide essential data for developing interventions to reduce burnout and boost work engagement among nurses in INS settings.
