    Summary

Vestibular neuronitis can cause prolonged vertigo and gait instability. Many patients experience persistent symptoms, with reduced caloric responses observed even after one year, indicating potential long-term vestibular dysfunction.

    Area of Science:

    • Neurology
    • Otolaryngology
    • Vestibular System Disorders

    Background:

    • Vestibular neuronitis is a common cause of vertigo.
    • Long-term outcomes and neurotological findings require further investigation.

    Purpose of the Study:

    • To evaluate the long-term neurotological follow-up of patients with vestibular neuronitis.
    • To assess the persistence of symptoms and objective findings.

    Main Methods:

    • Neurotological examinations including spontaneous nystagmus, positional nystagmus, caloric tests, and galvanic body sway test (GBST).
    • Follow-up observations over extended periods (up to 9 years and 4 months).
    • Assessment of subjective symptoms such as vertigo, floating sensation, and gait unsteadiness.

    Main Results:

    • Vertigo subsided in 50% of cases within 3 months, but 6 subjects had persistent dizziness for over 1 year.
    • Spontaneous nystagmus persisted in 50% of cases after 1 year.
    • All tested cases showed reduced caloric responses, with 5 cases not recovering even after 1 year.

    Conclusions:

    • Vestibular neuronitis can lead to persistent vertigo and gait disturbances in a significant minority of patients.
    • Objective neurotological findings, particularly reduced caloric responses, may persist long-term.
    • Further research is needed to understand the mechanisms behind persistent symptoms and vestibular dysfunction.
