A 45-year-old man with a history of patent ductus arteriosus (PDA) presented with dual vegetations on the aortic valve and PDA. This case highlights the importance of comprehensive imaging and a multidisciplinary approach in managing complex infective endocarditis, demonstrating successful surgical outcomes.
