Simulation-Based Training for PAs: Improving Pediatric Surgical Skills in Rural Hospitals

Objective:
To evaluate the effectiveness of a simulation-based training program in enhancing pediatric surgical knowledge, technical skills, and team dynamics among physician assistants (PAs) in a rural hospital setting.
Design:
A pilot study integrating simulation-based training into a structured onboarding curriculum for newly graduated PAs. The program combined high-fidelity simulations, didactic sessions, and progressive learning. Pre- and postsimulation questionnaires assessed participants' knowledge, technical competency, and confidence. Data were analyzed using quantitative and qualitative methods.
Setting:
The study was conducted in a rural hospital with a high-fidelity simulation laboratory to replicate pediatric surgical emergencies and trauma scenarios.
Participants:
The training program included 16 healthcare professionals, comprising newly graduated PAs, nurse practitioners, preoperative nurses, operating room nurses, and surgical technicians. All participants completed the training.
Results:
Postsimulation evaluations revealed significant improvements in participants' confidence (p < 0.05), technical skills, and teamwork. Participants demonstrated enhanced capabilities in managing airway obstruction, identifying postoperative bleeding, performing tube thoracostomy, and implementing massive transfusion protocols. Qualitative feedback emphasized the value of hands-on simulation in improving crisis preparedness and reinforcing multidisciplinary collaboration.
Conclusions:
Simulation-based training effectively improves clinical competency, confidence, and crisis management skills among PAs in pediatric surgical care. Structured onboarding programs integrating simulation and didactic components address critical training gaps in rural hospitals and enhance patient care outcomes.
