Background:

  • Postpartum chronic pain is a significant issue affecting numerous women after childbirth.

Purpose of the Study:

  • To determine the prevalence and identify risk factors for chronic pain 6 months postpartum.
  • To develop and validate a machine learning model for predicting postpartum chronic pain.
  • To enhance clinical decision-making and patient care through predictive analytics.

Main Methods:

  • A cohort of 1,398 postpartum women was analyzed from July 2021 to June 2022.
  • Six machine learning algorithms were benchmarked using nested resampling to evaluate classification error (CE).
  • The extreme gradient boosting model was selected for its superior performance in predicting chronic pain.

  • 383 out of 1,398 women (27.4%) developed chronic pain 6 months postpartum.
  • Five key predictors were identified: pain at 3 days postpartum, pre-delivery BMI, newborn weight, multiparous delivery, and gestational back pain.
  • The extreme gradient boosting model achieved the highest performance with a CE of 0.147 and F1 score of 0.851.

Conclusions:

  • The extreme gradient boosting algorithm effectively predicts postpartum chronic pain.
  • Incorporating five identified risk factors into the model enhances predictive accuracy.
  • This ML model offers a promising tool for early identification and management of postpartum chronic pain.
