Background:

  • Breast cancer (BC) is a leading malignancy in Indian women.
  • Understanding health-related quality of life (HRQoL) evolution in BC survivors is vital due to high survivorship.
  • Limited longitudinal data exists on HRQoL for Indian BC patients.

Purpose of the Study:

  • To prospectively assess the longitudinal changes in HRQoL among Indian breast cancer patients.
  • To identify predictors of HRQoL trajectories in this population.
  • To provide insights into survivorship care for breast cancer in India.

Main Methods:

  • Prospective observational study of 500 Indian breast cancer patients (June 2019-March 2022).
  • HRQoL assessed using EORTC-QLQ-C30, EORTC-QLQ-BR-23, and EQ-5D-5L at pre-treatment, post-treatment, and 6-month follow-up.
  • Linear mixed-effects modeling used to analyze HRQoL trends and influencing factors.

Main Results:

  • Most patients (60.8%) had locally advanced disease and received multimodal therapy.
  • Global QoL showed a slight decline, but social, cognitive, and role functions improved significantly.
  • Body image and sexual enjoyment deteriorated; fatigue increased with radiotherapy.
  • Younger age, higher education, and financial independence predicted better HRQoL.
  • Social displacement negatively impacted QoL trajectories.

Conclusions:

  • This is the first longitudinal HRQoL study in Indian BC patients, demonstrating early survivorship improvements in most functional domains.
  • Sociodemographic factors (age, social displacement, education, financial independence) are stronger HRQoL predictors than treatment modalities.
  • Further long-term follow-up is recommended for a comprehensive assessment of HRQoL in Indian BC survivors.
