Inferior Wall Myocardial Infarction Due to Shotgun Pellet Embolization

Myocardial ischemia resulting from penetrating trauma is uncommon but can occur due to shotgun injuries. These injuries may cause direct trauma to coronary arteries or result in pellet embolization.

A 21-year-old man with a history of shotgun injury presented with epigastric pain. Imaging revealed a pellet lodged in the right coronary artery without significant pericardial effusion. Due to recurrent episodes of chest pain and pericarditis despite optimal medical therapy, he underwent foreign body removal and coronary artery bypass grafting.

Although cases of medically managed shotgun pellet embolization have been reported, complications may require surgical interventions.
