    • Four cases of granuloma causing tracheal obstruction were analyzed.
    • All patients underwent tracheostomy with a fenestrated tube insertion.

    Findings:

    • The insertion of fenestrated tubes was identified as the direct cause of excessive granuloma formation.
    • This excessive granuloma led to significant tracheal obstruction in the presented cases.

    Implications:

    • Fenestrated tubes may pose a risk for granuloma development and subsequent tracheal obstruction.
    • Clinical vigilance is required to monitor for and manage granuloma formation after fenestrated tube use.
