    Background:

    • Balo's concentric sclerosis is a rare demyelinating disease of the central nervous system.
    • Its characteristic lesions are concentric rings of demyelination and remyelination.
    • The exact nature and developmental stage of Balo lesions remain debated.

    Observation:

    • Postmortem examination of a 54-year-old woman revealed widespread Balo lesions.
    • Lesions were identified in the brain and spinal cord, the latter being a novel finding.
    • Histological and ultrastructural analyses detailed the lesion composition.

    Findings:

    • Balo lesions exhibit alternating bands of intact and demyelinated myelin, centered on perivascular inflammation.
    • The lesion center represents the oldest area, with concentric rings decreasing in age outwards.
    • Intact myelin bands primarily consist of remyelinated fibers, resembling active multiple sclerosis plaque margins.

    Implications:

    • The findings suggest Balo's concentric sclerosis may represent an intermediate stage of multiple sclerosis lesion development.
    • The presence of spinal cord lesions expands the known anatomical distribution of Balo's disease.
    • This study provides new insights into the pathogenesis and evolution of demyelinating disorders.
