The Colorado State Innovation Model successfully integrated behavioral health services into primary care practices. Key components of high-performing primary care practices mediated improvements in behavioral health quality measures.
Area of Science:
- Primary Care
- Health Services Research
- Behavioral Health Integration
Background:
- The Colorado State Innovation Model (SIM) initiative focused on enhancing primary care practices.
- It aimed to support the integration of behavioral health services and advanced primary care models.
- This project specifically evaluated the implementation of these integrated services and the core components of high-performing primary care.
Purpose of the Study:
- To evaluate the implementation of behavioral health integration within primary care settings.
- To assess the adoption of the Building Blocks of High Performing Primary Care framework.
- To determine the impact of these implementations on clinical quality measures.
Main Methods:
- 334 primary care practices participated in the SIM initiative across three cohorts.
- Practices received support for implementing advanced primary care and behavioral health integration.
- Data were collected using the Comprehensive Primary Care Practice Monitor and quarterly behavioral health clinical quality measures.
Main Results:
- Practices demonstrated significant improvements across all evaluated building blocks of high-performing primary care (P < .0001).
- Onsite behavioral health integration was achieved by 78% of practices.
- Significant improvements were observed in depression screening and monitoring, including for maternal depression (P < .0001 and P < .001, respectively).
Conclusions:
- The SIM initiative facilitated successful implementation of behavioral health integration and high-performing primary care components.
- Implementation of foundational primary care building blocks was shown to mediate improvements in clinical quality measures.
- These findings highlight the effectiveness of integrated models in improving behavioral health outcomes in primary care settings.
