    Summary

    Fifteen of 172 hairy cell leukemia (HCL) patients developed a second malignancy, most commonly skin and lung cancers. HCL patients with unusual features warrant investigation for secondary cancers.

    Background:

    • Hairy cell leukemia (HCL) is a rare B-cell chronic lymphoid leukemia.
    • The occurrence of secondary malignancies in HCL patients is not well-documented.
    • Understanding the spectrum of second cancers in HCL is crucial for patient management.

    Purpose of the Study:

    • To investigate the incidence and types of second malignancies in patients with hairy cell leukemia.
    • To analyze the clinical characteristics of HCL patients who develop secondary cancers.
    • To determine the temporal relationship between HCL diagnosis and the development of second neoplasms.

    Main Methods:

    • Retrospective review of 172 patients diagnosed with HCL over a 10-year period.
    • Identification and classification of all second malignancies diagnosed in the HCL cohort.
    • Comparison of clinical features between HCL patients with and without second malignancies.

    Main Results:

    • Fifteen out of 172 HCL patients (8.7%) were diagnosed with a second malignancy.
    • The most frequent second malignancies were skin cancers (basal cell carcinoma, squamous cell carcinoma, melanoma) and lung carcinoma.

    Conclusions:

    • Hairy cell leukemia patients have an increased risk of developing secondary malignancies, particularly skin and lung cancers.
    • The clinical presentation of HCL in patients with second malignancies does not differ from the general HCL population.
    • Further investigation for second malignancies is recommended in HCL patients presenting with unusual disease features.
