In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. Background:

  • The relationship between glaucoma and autonomic nervous system (ANS) function is not fully understood.
  • Primary open-angle glaucoma (POAG) and exfoliation glaucoma (EXG) have distinct pathologies that may influence ANS function differently.

Purpose of the Study:

  • To investigate the association between glaucoma subtypes (POAG, EXG) and time-domain heart rate variability (HRV) parameters.
  • To compare HRV parameters, including standard deviation of normal-to-normal intervals (SDNN), root mean square of successive differences (RMSSD), and coefficient of variation of R-R intervals (CVRR), between glaucoma patients and controls.

Main Methods:

  • Included 809 eyes from 809 patients with POAG, EXG, and controls.
  • Measured HRV using fingertip sphygmography.
  • Analyzed time-domain HRV parameters (SDNN, RMSSD, CVRR) and performed linear regression.

Main Results:

  • A significant difference in CVRR was observed among the groups (p=0.010).
  • Exfoliation glaucoma (EXG) patients exhibited significantly lower SDNN and CVRR compared to primary open-angle glaucoma (POAG) patients (p=0.0156, p=0.0037).
  • Highest intraocular pressure (IOP) was significantly associated with CVRR in multivariate analysis.

Conclusions:

  • HRV parameters may indicate autonomic nervous system alterations specific to different glaucoma subtypes.
  • Findings suggest distinct ANS profiles in POAG and EXG, potentially linked to intraocular pressure.
