Background:

  • Pineal region cavernoma is a rare diagnosis (<1% of cerebral cavernous malformations) with a female predominance.
  • These benign vascular malformations are curable, distinguishing them from other pineal region lesions.

Observation:

  • A 40-year-old female presented with headache and vomiting, diagnosed with pineal cavernoma.
  • The patient was effectively managed with a ventriculoperitoneal shunt for hydrocephalus.

Findings:

  • MRI is effective for diagnosing pineal region cavernous lesions.
  • Pineal cavernoma carries a high risk of rebleeding and can cause acute obstructive hydrocephalus.

Implications:

  • Early diagnosis of pineal cavernoma is critical.
  • Treatment of hydrocephalus is essential, especially when surgical intervention is not feasible.
  • A palliative approach can be a viable management option for pineal cavernoma.
