Background:

  • Atrial fibrillation (AF) is a common arrhythmia with high recurrence rates post-ablation.
  • Insulin resistance is increasingly recognized as a factor influencing cardiovascular health.
  • Previous research suggests a link between insulin resistance markers and AF recurrence.

Purpose of the Study:

  • To systematically review and meta-analyze studies on surrogate insulin resistance markers and AF recurrence after ablation.
  • To assess the predictive value of various insulin resistance markers for post-ablation AF recurrence.

Main Methods:

  • Systematic review and meta-analysis of observational studies.
  • Searched PubMed, Web of Science, Embase, and Scopus up to August 2024.
  • Included 9 studies with 7729 participants; used random-effects model and Newcastle-Ottawa scale for bias assessment.

Main Results:

  • Triglyceride-glucose index (HR 1.29), HOMA-IR (HR 1.27), and MIRSI (HR 1.04) were significantly associated with AF recurrence.
  • Triglyceride-to-HDL ratio showed no significant association (HR 1.09).
  • Low heterogeneity observed for HOMA-IR and MIRSI.

Conclusions:

  • Surrogate markers of insulin resistance are valuable predictors of AF recurrence post-ablation.
  • These markers may aid in identifying patients for targeted preventive strategies.
  • Further prospective studies are needed to confirm these findings and their clinical utility.
