Background:

  • Acute pancreatitis (AP) in dogs presents variably, complicating prognosis.
  • Accurate prognostication is crucial for effective management of canine AP.

Purpose of the Study:

  • To compare the predictive accuracy of five scoring schemes for death and prolonged hospitalization (≥5 days) in dogs with AP.
  • To assess the concordance of each scoring scheme with these outcomes.

Main Methods:

  • A retrospective study of 504 client-owned dogs diagnosed with AP.
  • Data included signalment, history, physical exam, diagnostics, hospitalization length, and survival.
  • Five prognostic schemes (OS, CSI, APPLEfull, CAPS, MCAI) were calculated for each dog.

Main Results:

  • Overall concordance of scoring schemes with outcomes was low.
  • The APPLEfull and MCAI scores showed significant differences between survivors and non-survivors.
  • APPLEfull demonstrated the highest concordance (0.632) with hospitalization length, followed by MCAI (0.576).

Conclusions:

  • APPLEfull and MCAI offer the best population-level predictive discrimination for hospitalization duration in canine AP.
  • Individual predictions based on scoring schemes should be interpreted cautiously due to low case fatality rates.
