    Background:

    • Basal cell carcinoma (BCC) is the most common type of skin cancer, rarely metastasizing.
    • Metastatic BCC, particularly to the lungs, is exceptionally rare, with limited documented cases.

    Observation:

    • A 47-year-old Japanese male presented with a chronic cheek ulcer that progressed to basal cell carcinoma.
    • The patient developed regional lymph node metastases followed by lung metastases.
    • The patient experienced massive hemoptysis and succumbed to the disease.

    Findings:

    • This represents the 38th reported case of basal cell carcinoma with lung metastases globally.
    • It is the third documented occurrence in a Japanese patient.
    • Histological examination confirmed basal cell carcinoma in both the primary tumor and metastatic sites.

    Implications:

    • This case underscores the potential for aggressive behavior and distant metastasis in basal cell carcinoma, even in rare presentations.
    • It emphasizes the need for vigilant follow-up and consideration of metastatic disease in patients with advanced or recurrent BCC.
