1Division of Urology, Graduate School of Medical and Dental Sciences Niigata University Niigata Japan.
A rare case of a perianal abscess in a 14-year-old boy was linked to a urethral foreign body present for 15 months. This highlights the need for vigilance in patients with cognitive impairments presenting with urinary issues.
Area of Science:
- Urology
- Pediatric Surgery
- Radiology
Background:
- Foreign body insertion into the urethra can lead to serious complications.
- Long-term retention of urethral foreign bodies is uncommon but can result in significant morbidity.
- Intellectual disability may increase the risk of foreign body insertion and complicate diagnosis.
Purpose of the Study:
- To report a unique case of perianal abscess secondary to a long-standing urethral foreign body.
- To emphasize the diagnostic challenges and management of such cases in pediatric patients with cognitive impairments.
- To raise awareness among medical professionals regarding potential underlying behavioral issues in refractory urinary tract conditions.
Main Methods:
- A 14-year-old male with intellectual disability presented with perianal symptoms.
- Imaging studies, including CT and MRI, were used to identify the foreign body.
- Cystourethroscopy with grasper was performed for foreign body removal.
Main Results:
- A urethral foreign body, an allene fiber broom ear, was present for 15 months.
- The foreign body was successfully removed via cystourethroscopy.
- The case highlights a delayed presentation of perianal abscess linked to the urethral foreign body.
Conclusions:
- Healthcare providers should consider urethral foreign bodies in patients with intellectual disability presenting with lower urinary tract symptoms or perianal infections.
- Vigilance in screening for and treating cognitive and behavioral problems is crucial in managing refractory urinary tract inflammation or hematuria.
- Prompt diagnosis and removal of urethral foreign bodies are essential to prevent complications such as abscess formation.
