    Summary

    Preoperative optical coherence tomography (OCT) can identify key indicators for idiopathic epiretinal membrane (ERM) surgery. Ellipsoid zone disruption and retinal nerve fiber layer schisis predict better visual outcomes after surgery.

  • Ellipsoid zone (EZ) disruption on OCT predicted improved BCVA.
  • Focal retinal nerve fiber layer (RNFL) schisis on OCT predicted improved BCVA.
  • ERM stage, foveal layers, herniation, thickness, cysts, holes, and SUKIMA gap did not predict outcomes.
  • Conclusions:

    • Preoperative OCT findings of EZ disruption and RNFL schisis can identify patients likely to experience significant visual improvement after ERM surgery.
    • These biomarkers aid in surgical decision-making and patient counseling.
