    Background:

    • Intra-articular pressure is a critical factor in hip joint health.
    • Various pediatric hip conditions, including Perthes' disease and synovitis, can affect intra-articular pressure.

    Purpose of the Study:

    • To measure and compare intra-articular pressure in children with different hip pathologies.
    • To determine optimal immobilization positions for synovial effusion to prevent femoral head compromise.

    Main Methods:

    • Intra-articular pressure was measured in 94 hips across 81 children.
    • Children presented with Perthes' disease, transient synovitis, septic arthritis, reactive arthritis, and arthritis with urticaria.

    Main Results:

    • Elevated pressures were recorded in all studied conditions, ranging from 3.4 kPa (Perthes' disease) to 32.3 kPa (arthritis with urticaria).
    • Transient synovitis showed significantly lower pressure in flexion (2.3 kPa) compared to neutral/internal rotation (26.6 kPa).

    Conclusions:

    • Immobilization in 30-45 degrees of flexion with slight external rotation is recommended for hip synovial effusion.
    • Neutral or extended positions may compromise femoral head circulation.
