Summary

Persistent fatigue is common after transient ischemic attack (TIA). Early fatigue within 14 days predicts lasting symptoms up to 12 months, independent of acute lesions but linked to prior anxiety or depression.

Background:

Purpose of the Study:

  • To investigate the prevalence and trajectory of fatigue up to 12 months after TIA.
  • To identify factors characterizing patients experiencing pathologic fatigue post-TIA.

Main Methods:

  • Prospective cohort study of TIA patients from a specialized stroke unit.
  • Fatigue assessed using Multidimensional Fatigue Inventory (MFI-20) and Fatigue Severity Scale at 14 days, 3, 6, and 12 months.
  • Linear regression models used to analyze associations between prognostic factors and 12-month fatigue.

Main Results:

  • Included 354 patients; 287 provided baseline data (mean age 70.0 ± 11.1).
  • Pathologic fatigue (MFI-20 General Fatigue ≥12) affected 61.3% at baseline, remaining around 53-54% at 12 months.
  • Previous anxiety/depression was twice as common in fatigued patients; no association with acute ischemic lesions was found.

Conclusions:

  • Pathologic fatigue is a common and persistent issue for up to 12 months post-TIA.
  • Early fatigue (within 14 days) is a strong predictor of continued fatigue.
  • Previous anxiety/depression, not acute ischemic lesions, appears associated with persistent fatigue.
