    Summary

    Severe iron deficiency anemia can rapidly worsen diabetic retinopathy, even in patients with good vision. This progression may necessitate advanced treatments like panretinal photocoagulation and pars plana vitrectomy.

    Background:

    • Diabetic retinopathy is a common complication of diabetes mellitus.

    • Three patients with diabetes mellitus (average 17 years) presented with good vision and mild to moderate background diabetic retinopathy.
    • These patients developed severe iron deficiency anemia due to various causes.
    • Their diabetic retinopathy rapidly progressed to a severe proliferative phase.

    Findings:

    • Severe iron deficiency anemia is a significant risk factor for rapid diabetic retinopathy progression.
    • The progression occurred despite initial good visual status and mild retinopathy.
    • Advanced ocular interventions were required in two patients.

    Implications:

    • Early detection and management of iron deficiency anemia are crucial in diabetic patients.
    • Anemia can unmask or accelerate underlying diabetic eye disease.
    • This highlights the complex interplay between systemic health and ocular complications in diabetes.
