Background:

  • Penicillins are common allergy triggers, but diagnosis is complex.
  • Current diagnostic work-up involves skin testing (ST) and drug provocation tests (DPT).
  • Faster delabelling strategies are needed, especially in European adults.

Purpose of the Study:

  • To evaluate the safety of direct DPT without prior ST in European adults with low-risk penicillin allergy.
  • To analyze the role of ST and T-cell involvement using lymphocyte transformation tests (LTT).

Main Methods:

  • Prospective evaluation of 269 penicillin-allergic patients in Málaga, Spain (2023).
  • Patients had non-immediate reactions or unknown reactions.
  • Direct single-dose DPT was performed; positive cases underwent ST and LTT.

Main Results:

  • Only 5.9% of patients had positive DPT, primarily to amoxicillin or amoxicillin-clavulanic acid.
  • Positive DPT reactions were mostly mild maculopapular exanthema.
  • ST was positive in 19% of DPT-positive cases, while LTT was positive in 86.7%.

Conclusions:

  • Direct single-dose DPT is a safe method for penicillin allergy delabelling in selected adult patients.
  • ST has limited diagnostic value in this population.
  • LTT effectively confirms T-cell mediated hypersensitivity in penicillin allergy.
