Abstract:
Heart failure is a chronic disease with a high prevalence associated with the comorbidities of aging. A heart failure health care pathway has been set up, based on an organizational innovation involving the appointment of a pathway nurse coordinator. The comprehensive care provided includes systematic screening for frailty. A total of 51 patients were included and 428 care needs identified, including 149 frailties, justifying the relevance of the heart failure health care pathway.
